The application and contract below is for use by any module group and other exhibitors such as train clubs and historical societies, who are not selling merchandise or anything else other than memberships to their respective group.

The blank version of the application is a regular pdf file that you print, fill in manually and return via U.S. mail.

The electronic version of the application is a pdf data entry form, allowing you to type in the requested information. When completed, simply print it and return via U.S. mail.

NOTE: The electronic version should be downloaded and opened using Adobe Acrobat Reader. Browser plugins that directly open the file in the browser, and generic pdf readers that are installed on some computers, will likely not display the file correctly.
